COMMUNITY OUTREACH WORKER

PURPOSE OF CLASSIFICATION: The purpose of this classification is to provide outreach awareness and education to TENNderCare recipients and families. Duties include, but are not limited to: conducting home visits; participating in community and outreach activities; completing and maintaining accurate records of program activities; and performing additional duties as assigned by the Program Manager.
ESSENTIAL FUNCTIONS: The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. Conducts home visits for TennCare children identified as non-compliant with their EPSDT screenings. Works with community agencies, schools, churches, daycares, local businesses, and local physicians to access TennCare families. Completes and maintains accurate records of program activities, while adhering to all health department HIPPA protocols. Conducts TENNderCare programs and activities within schools, daycares, faith based organizations, shelters and Head Starts. Conducts community outreach activities for TennCare eligible children and their families. Provides individual and/or group education about the programs by contacting eligible TennCare families either by phone, written notification or face to face. Provides TENNderCare programmatic updates for health and medical professionals. Conducts outreach at health fairs, parents meetings, business partners and other events. Prepares all educational materials and other resources needed by the TENNderCare program. Conducts presentations to health professional, parents, community groups, business partners and children. Participates in community activities including health fairs, community events, faith-based activities, etc. where target population can be reached. Attends all trainings for job readiness/preparedness and continuation of training deemed necessary by the program manger.
ADDITIONAL JOB FUNCTIONS: Responds to and reports immediately if called upon by local, regional or central office supervisors, as part of a coordinated emergency response by the Department of Health.
Performs other related duties as required.
MINIMUM TRAINING AND QUALIFICATIONS: Must be a Knox County resident (minimum of one (1) year), High School Diploma or GED; or any equivalent combination of education, training, knowledge, and experience which provides the requisite knowledge, skills, and abilities for this position. Requires the use of personal vehicle, and must possess and maintain a valid Tennessee driver’s license with proof of auto insurance coverage. Candidates must be bilingual in Spanish and English and have the ability to communicate in both verbal and written forms.
PHYSICAL REQUIREMENTS: Must be physically able to operate a variety of office machines and equipment such as personal computers, printers, telephones, photocopiers, scanner, fax machines, laminators and etc. Must be able to use body members to work, move or carry objects or materials up to approximately 25 lbs. Physical demand requirements are at levels of those for sedentary work.
SALARY: $12.50 per hour
SCHEDULE: Must be able to work twenty (20) flexible hours (between 8:00-4:30 p.m.) per week. Occasional work hours may include evenings and weekends.
